I just watched the movie Drive, and while I really liked the movie I thought the soundtrack was terrible. Every time the music kicked in it took away from the scene where a basic song or just silence would have done the trick.
I'd also put Scarface on this list. I thought Heat was in this boat too, but I've gotten some people saying it wasn't that bad so I have to rewatch that.
I am sure there are more. I'd put 80's porn on the list, but your mileage on that might vary.
I remember going to see Rocky IV at the theater and being really mad that the awesome Bill Conti orchestra music that made the Rocky movies great was replaced by current 80's pop songs.

You know, Conti is awesome, but damn if the Rocky IV soundtrack isn't the best 80's soundtrack of all time. Totally fits the over the top style of the movie.
Scarface was what I immediately thought of when I read the title.
I'm tempted to say the Dark Knight. I like the score and all but really Elfman's score is classic.

I can't think of any great movies with particularly bad soundtracks, but both Strange Days and Lost Highway had great soundtracks while the movies left much to be desired. In fact, I only watched Lost Highway because I like the music so much. I was sorely disappointed.
