Right now I'm leaning toward work. Interesting that his music just happened to be stopped right after the attack. Was the music man waiting? And not a whole lot of reaction to it. But most of all, that "biceps to the balls" move is very rarely seen outside of pro wrestling. If I'm going for an attack for real, I don't think that's the move I use.
Plus there's the fact that they, y'know, posted the YouTube video of it up on WWE.com. If this were an actual fan attack, they'd just be encouraging future incidents. By putting it on their website, they're making it part of "the WWE Universe", and will no doubt make it part of a storyline.

I'd doubt. A crazy moron tried to do a wrestling spot and incompetent local security let him. They're lucky he didn't have a knife dipped in AIDS or something. WWE canceled a meet & greet afterwards. Maybe it was the fan Punk punched and he'd been plotting his revenge.

Yeah, Orton just rolling with and selling an unexpected Vintage WWE Main Event Style Nutshot until he realized what was going on is maybe the most mature, professional thing he's done. A true reminder of and peak to the superior worker version of Orton who debuted in late 2011 during the Mark Henry feud.
Anyway, this better lead to an Orton vs. Justin Gabriel/Leo Kruger New Truth Commission feud, which will henceforth be referred to as DISTRICT RANDY.
