I wasn't sure exactly how to title this thread, but here is my issue. I won a cbs-sportsline fantasy baseball league and to claim my winnings I have to fill out this form ... Click Here (12603254501.baseball.sportsline.com)
It seems to ask for a lot of information. Is it fishing for extra information? I do want to claim the money, so if I have to fill it all out then so be it. I'm just curious if anyone knows anything more about these before I go and fill it out.
I wouldn't mind filling the form out, except for the SS#. I don't doubt their intentions, just their security. Maybe they would accept a photocopy of your driver's license instead? It wouldn't hurt to look back at the rules of the game and see if they listed any particular information as a requirement.
edit: Never mind. Dr. Dirt explained why they need the SS#. I would do it, but I wouldn't have to like it....

They are going to send in a 1099 (think that's the #) and need your SSN for the IRS. The started asking for that at the state fair here so you have to report any winnings on your taxes. Our daughter won $82 (won the youth bread division) and they wont cut a check unless they have the SSN. So my daughter's win will bump us up a bracket.
I agree. The only series I would get on DVD are either British shows (i.e. Coupling, The Office), shows that aren't in reruns anymore (i.e. Soap) or dramas, which rarely make basic cable syndication anyway (i.e. 24).
