I didn't see it, but a few months back Lance Storm forgot to kick out, and Funaki got a win.
Another one I can't remember if I saw or not: Hooker, over at Weekly Visitor, used to talk of a time when Rikishi sold a low blow by "trying to fly away." I have a vague memory of this.
One more: There was a deathmatch in FMW or some other fed, and the explosives turned out to be duds.

Scott Hall is fighting Sting. Hall hits a big move, goes for the cover. Sting is supposed to kick out right before the ref's hand hits the canvas for the three count. No biggie, right?
Except Sting forgets to kick out.
So the ref's hand hits the canvas for the third time, THEN Sting kicks out.
Hall gets up, doing the typical "I just won, you blind idiot!" pantomimes. And the following exchange occurs:
REF: *holds up two fingers* TWO! HALL: *holds up four fingers* FOUR!
Nearly fell outta my freaking chair laughing.
